Position overview * Employment Type: Full Time, 80 hours per fortnight with a monthly ADO (ongoing) * Location: Warrnambool Campus (community-based role with regular RACF and inpatient interface) * Classification: Clinical Nurse Consultant * Salary Range: $118,149.20 – $124,368.40 pro rata, plus superannuation (dependent on skills and experience) * Flexibility: Applications from candidates seeking part‑time employment will be considered About the role South West Healthcare (SWH) is seeking an experienced and compassionate Clinical Nurse Consultant - Care of Older Adults (Residential In Reach) to join our Community Rehabilitation Service. This role has a strong and primary focus on the Residential In Reach (RIR) service, delivering expert nursing care to residents of Residential Aged Care Facilities (RACFs) who are experiencing acute or sub‑acute clinical deterioration. The aim of the service is early intervention, hospital avoidance, and timely care in the resident's usual place of living, while also supporting older inpatients within SWH who are experiencing delirium, frailty, or cognitive impairment. Working autonomously and collaboratively, you will partner closely with GPs, Hospital in the Home (HITH), emergency and inpatient teams, RACF staff, residents and families to ensure coordinated, person‑centred care and smooth transitions across settings. Key focus - Residential In Reach The Residential In Reach component of this role is central and includes: * Proactive nursing assessment and intervention for RACF residents with deteriorating health status * Rapid clinical response to prevent unnecessary Emergency Department presentations and hospital admissions * Collaboration with GPs and medical teams to enable care in the community, including referral to Hospital in the Home (HITH) * Early identification and escalation of clinical deterioration using evidence‑based practice * Supporting RACFs through education, capacity building, and clinical leadership * Facilitating timely transfers back to RACFs when hospital admission has occurred What you will bring The successful applicant will demonstrate: * Advanced clinical expertise in aged care, gerontology, acute or subacute nursing * High‑level assessment skills for older people with complex and changing health needs * Proven experience in Residential In Reach, community, subacute, or hospital‑avoidance models of care * Strong communication skills and confidence working collaboratively with GPs, RACFs, inpatient teams and families * A clear commitment to person‑centred, wellness and reablement‑focused care You will be able to: * Conduct comprehensive, evidence‑based geriatric assessments * Recognise and respond to delirium, dementia, frailty and cognitive impairment * Deliver timely triage and treatment aligned with Residential In Reach referral pathways * Ensure care plans are clearly documented, actioned, and communicated to all stakeholders * Advocate for residents' goals of care, preferences and dignity * Support smooth transitions between aged care, community and inpatient settings Inpatient clinical leadership In addition to Residential In Reach, you will: * Provide expert clinical leadership and consultation within SWH inpatient settings * Support patients experiencing delirium, cognitive impairment and frailty * Provide education and guidance to staff on best‑practice delirium and dementia care * Contribute to service improvement initiatives aimed at reducing avoidable deterioration and admissions What we offer You will enjoy benefits such as: * Excellent salary packaging options including meals & entertainment, novated leasing and additional superannuation contributions * Internal and external professional development opportunities * Ongoing clinical supervision * Excellent terms and conditions of employment Who we are As an organisation SWH prides themselves on their values; Care, Respect, Excellence, Integrity and Leadership and their overall dedication to the local community.
