About the Role:

* Provide holistic, client-focused care in line with each client's tailored care plan.
">
* Assist clients with daily living activities and ensure their comfort in their own homes.
">
* Work closely with Registered Nurses to deliver client-specific care, including personal hygiene, toileting, nutrition, mobility, and more.
">
* Identify client care needs and work collaboratively with the client's care team to ensure high-quality, holistic care.
">
">
Key Responsibilities:
">
">
* Provide personal care and assist with daily activities, following the Client Care Plan.
">
* Communicate effectively with clients, their families, and team members.
">
* Support clients with light housekeeping duties to promote independence, including laundry, cleaning, and meal preparation.
">
* Maintain accurate daily client notes and promptly report any concerns to the RN/Care Manager.
">
">
Qualifications:
">
">
* Cert III in Aged Care/Disability (or equivalent).
">
* Current Senior First Aid (HLTAID011), CPR (HLTAID009) and Manual Handling (HLTWHS005) (with 3+ months until expiry).
">
* A roadworthy and insured vehicle with a current driver's licence.
">
* Federal Police Clearance (within the last 6 months).
">
