Support Worker - Central Coast North region Permanent Part-time: 24 - 36 hours per week Stand up active working shift 24/7 roster The roles provide support and work in partnership with people living with disability to live within the community in the way that they want to. Your responsibilities: 1. Collaborate with customers to develop individualised person-centered support plans that incorporate their identified wellbeing needs, strengths and goals. 2. Provide support to customers to improve their independence by assisting with skill development, transport, social activities, supporting with finances, attending appointments, cooking, medication and personal care. 3. Support and encourage our customers to participate in activities of their choice, such as: social, leisure, sporting, educational and employment activities 4. Keep customer records up to date on a daily basis using online systems and electronic databases. 5. Develop and maintain professional relationships and networks with customers, families, caregivers and key stakeholders. 6. Support customers to navigate the sector to access any additional service they may require. 7. Seek out innovative ways to enhance the recovery journey and overall customer experience with New Horizons. Minimum Certificate III / Cert IV in Disability, Community or Mental Health and/or willingness to obtain. 2. A strong set of values that guides your work, incorporating our organisational values of passion, integrity & respect. 3. Great computer skills with proven ability to use Microsoft Office and customer relationships management systems.
