We’re expanding and are looking for a dedicated Registered or Enrolled Nurse to join our Access Aged Care team in Mandurah. Supportive culture: We value your ideas and back your professional development. What You’ll Do Work alongside RACF care teams as the on site Access Aged Care nurse. Facilitate GP rounds (both in-person and telehealth) to ensure smooth, timely care. Prepare, organise, and triage referrals to maximise efficiency and outcomes. Conduct bedside assessments and clinical reviews. Act as the communication bridge between RACFs, GPs, and families. Deliver excellent customer service, with empathy and professionalism at every interaction Build strong relationships with residents, families and facility partners. What You’ll Bring Diploma/Bachelor of Nursing and AHPRA registration. Minimum 18 months experience as a Registered or Enrolled Nurse. Aged Care experience (desirable). Strong organisational, problem-solving and communication skills. Confidence with IT and documenting resident needs. A driver’s licence and reliable vehicle. Ready to Apply?
