* Conduct comprehensive patient assessments, considering physical, psychological, social, and cultural needs.
* Develop and evaluate individualized Care Plans in partnership with patients, families, caregivers, and healthcare professionals.
* Perform specialty-specific procedures and monitor treatment responses, adjusting care plans as needed.
* Communicate effectively with patients and families, providing education and support.
* Ensure accurate and timely documentation of all care activities.
* Advocate for ethical decision-making and coordinate interdisciplinary care plans.
Requirements
* 3-5 years' experience in a similar role.
* Experience working with individuals living with dementia across various care settings.
* Strong clinical skills in wound care, medication management, and chronic disease management.
* Excellent interpersonal and communication skills to engage with diverse stakeholders.
