The Aboriginal Mental Health Clinician (Community Transitions) will be part of a multidisciplinary team that works in partnership with the Custodial Mental Health Service, primary health networks, Community Mental Health Services and other government and non-government organisations (NGOs) to provide a collaborative approach to ensuring continuity of care.
