Clinical Wellbeing Manager, Community Services Role

We're seeking a Clinical Wellbeing Manager to join our team and make a real difference in the lives of clients across Mid North Coast region of Port Macquarie and Surrounding areas including South to Forster and north to South West Rocks.
This is a Full Time, 12-month Fixed-Term Contract opportunity to create positive impacts every day. As a member of our team, you will be at the forefront of transforming lives, providing tailored Home Care Package (HCP) care plans, leading multidisciplinary teams to deliver therapy, services, and equipment in the comfort of client homes.

Key Responsibilities:
* Be the champion for your clients, ensuring they are at the centre of every decision on their care journey.
* Create tailored HCP care plans, while leading a multidisciplinary team to deliver therapy, services, and equipment in the comfort of their homes.
* Coordinate day-to-day care delivery, including managing individualized budgets for clients.
* Assist in undertaking clinical pathways services.
* Provide support in the area and assist with changes related to the Support at Home program.
* Support clients on the Restorative Care Pathway Program as a Restorative Care Partner.
* Support clients requiring assessment and nursing care.
* Advocate for clients by participating in case conferences and clinical appointments to provide holistic support.

Requirements:
* Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
* Experience with continence assessments, catheter insertion/care and wound care.
* Tertiary qualifications in Case Management in Aged Care.
* Previous experience in case management, particularly in community or aged care settings.
* Strong networking skills to engage with both internal and external stakeholders.
* A current Australian driver's license and willingness to travel throughout the region daily.
* Full COVID-19 vaccinations highly desirable.
