The Social Worker in Older Persons Mental Health Service (OPMHS) of Southern Mental Health works in a dual role between the Community and Inpatient Mental Health Teams. Working as part of a multidisciplinary team the Social Worker contributes to consumers recovery, improved social functioning and affect a positive return to the community by delivering a range of clinical activities including mental health assessment, care planning, clinical review, psychosocial assessment, crisis management, clinical care coordination, advocacy and specialised therapy programs.
**About the role**:
Reporting to the Team Manager this role works collaboratively with the consumer and families/carers as well as liaising closely with a network of key stake holders, these may include - veteran and community mental health services and other government and non-government organisations.
Duties for the role will include:
- Care planning, clinical review, psychosocial assessment, crisis management, clinical care coordination, advocacy, and specialised therapy programs.
- Advocating for and negotiating on behalf of the consumer with relevant groups/organisations.
- Undertaking health promotion activities eg developing and implementing educational services as required.
**About you**:
- An appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
- Ability to provide core social work skills including psychosocial assessment, counselling, individual and group work, relationship and family interventions, crisis intervention and brief therapy.
- Must be highly motivated and have the ability to engage with consumers who have very high service needs.
