The Role

As the Clinical Nurse Consultant in SESLHD, you will contribute to POWH Renal Service by providing clinical service, consultancy, leadership, research, education, service planning and management. Our aim is to improve the health and well-being of patients with chronic kidney disease. We care for and support people with chronic renal disease from the moment they are diagnosed, which includes education, dialysis in hospitals and communities, kidney transplants, and ongoing support and care until the end of their lives.

As the CKD and Vascular Access CNC, you will use your significant expertise and qualifications to provide clinical leadership, coordination, consultation for CKD management and dialysis access in acute, satellite, and home dialysis units. You will map the CKD patient's journey to support treatment options, which includes triaging referrals, providing education, and arranging referrals for surgical consultations. You are responsible for scheduling patients and conducting CKD Clinics and Renal Vascular Access Clinic. You will ensure patients receive timely assessments, planning, and follow-up care. The CNC plays an important role in promoting home dialysis for eligible patients and encourages patients to pursue home dialysis by providing detailed information on the topic.

As the CNC you will develop, implement, and support quality improvement activities and research projects around CKD and dialysis vascular access particularly within a clinical risk management framework consistent with evidence-based practice and customer focused outcomes. You will be responsible for ongoing clinical supervision processes for Registered Nurses and deliver education to patients, carers and staff in a range of settings. As well as maintain your clinical and professional skills and actively participate in state and national working parties.

You will analyse current practices, evaluate the impact of new directions, initiate strategic changes to improve the delivery of care and optimise outcomes.

Where you'll be working

Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person-centred care.

Selection Criteria

* Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
* Having at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field. In addition, must have approved postgraduate nursing qualifications relevant to the field.
* Demonstrated ability to apply clinical expertise in a consultant role and the ability to work independently.
* Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
* Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidenced based nursing care.
* Demonstrated experience in leading and implementing change, and developing direction in clinical practice by initiation, utilisation, review and translation of evidence-based research into practice, within a person-centred framework.
* Demonstrated effective interpersonal skills, excellent written and verbal communication skills, advanced computer skills and use of software programs to analyse data and produce reports.
* Demonstrated ability to contribute to strategic and clinical service planning.
