Cabanda Care Inc is located in Rosewood a regional town located 20km west of Ipswich. Opening in 1987 as a 26-bed hostel, Cabanda was a vision of the local community to provide an aged care facility where people could age in the community they lived in.Since then, we have grown to include a 71 Bed Residential Aged Care Facility, a retirement village with 42 independent living units, Centre based Respite and Social Care Group, Home and Community Care, Meals on Wheels, Community Transport, and the Cabanda Café.Cabanda is the largest employer in Rosewood and the vision of Rosewood residents living in a community with those they have known all their lives has been maintained. Job Description
The Care Partner – Home Care plays a key role in coordinating the delivery of person-centred care that empowers individuals to live independently and with dignity in their own homes. This role is dedicated to supporting participants with varying care needs by facilitating high-quality, responsive, and respectful assistance aligned with their personal goals and preferences. The Care Partner works collaboratively with participants, families, and interdisciplinary teams to ensure care is tailored, consistent, and reflective of our Model of Care and Service. By fostering trusting relationships, promoting best practices, and encouraging continuous improvement, the Care Partner helps create a safe and supportive environment where participants remain at the centre of all care decisions and service planning.
Desired Skills and Experience
* Relevant experience in Home and Community Care, Case Management and/or Aged Care
* Demonstrated understanding and knowledge of participant needs from various funding programs (e.g. Home Care Packages, Support at Home, Commonwealth Home Support Programme)
* Proficiency conducting assessments, care planning, implementation, evaluation, and documentation and budget management of quality participant care in line with Quality of Care principles
* Strong communication skills to liaise with staff, participants, service providers, and health professionals, ensuring the delivery of customer-focused services
* Reflect a culturally safe, trauma aware and healing informed approach
* Ability to build strong rapport with older people, members of Cabanda Care Ltd and external providers through excellent communication skills (both verbal and written)
* Strong attention to detail, accuracy and computer literacy skills
* Ability to work autonomously and a commitment to collaborative teamwork and partnering
* Demonstrated analytical and problem-solving skills, and ability to manage competing priorities
* Must maintain a valid Police Check and hold a valid Queensland Driver's Licence and own vehicle
* Tertiary qualifications in Community or Aged Care service-related field such as a Certificate IV Community Services or Aged Care or equivalent (Desirable)
* Strong community links (Desirable)
* Experience with case management and rostering software (Desirable)
