Nurse Coordinator Chronic Diseases
About the Role:
The Department of Justice is seeking a dedicated and experienced Nurse Coordinator to join the health services team within the Western Australian Corrective Services system.
This role focuses on improving the health and wellbeing of incarcerated individuals with chronic conditions such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions.
Key duties include clinical leadership in care coordination, oversight, comprehensive assessments, development and monitoring of individual care plans, and support for self-management through education and culturally appropriate health promotion.
You will collaborate with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff to ensure a patient-centred approach.
You will contribute to discharge planning and continuity of care, including liaising with community health providers and Aboriginal health services where relevant.
You will also have opportunities to contribute to policy review and develop policies and procedures related to Chronic Disease Management.
Eligibility & Qualifications:
* Qualifications: You must be eligible for registration in the category of Registered Nurse by the Nursing and Midwifery Board of Australia.
* Permanent positions: You must be an Australian citizen or have permanent resident status in Australia.
* Special conditions: A valid C or C-A class driver's license or equivalent is required.
