OverviewBelong at Home is seeking enthusiastic, motivated and caring people to join our South West team as a Community Nurse/Clinical Care Partner (RN).
This is a permanent part-time role with the potential to transition into full time, offering flexibility and purpose in a community-focused setting.ResponsibilitiesCollaboratively support BaH clients approved for clinical service types under Support at Home.Support admissions of SaH clients into Belong at Home.Tasks associated with Clinical Care Partner defined within Support at Home program.Undertake clinical assessments of home care clients.Providing excellent care and service to clients in line with their personalised care plans.Monitoring chronic diseases and palliative conditions.Wound care, continence care, palliative care, medication management, restorative care and management of chronic diseases.A team player you will work closely with our care partners to ensure quality service delivery.Achieving the optimum care for clients by being responsive to their changing needs.Liaising with and referring to other organisations as required to provide for the clients needs.Assist the clinical leads with other duties as required.Qualifications & RequirementsCurrent AHPRA registration.Availability to work (M-F) – (ad hoc weekends where required).
Minimum 3 days a week.Aged Care experience is an advantage.Excellent verbal and written communication skills.A genuine enthusiasm, care and compassion in your commitment to working with the elderly.A National Police Clearance is required.A competitive remuneration package including salary packaging up to $15,900 per annum to reduce the amount of tax you pay.BenefitsSalary packaging up to $15,900 per annum; flexible, community-focused role; opportunity to transition to full-time.
