Care Coordinator Job Description
The primary responsibility of the Care Coordinator is to oversee, manage and deliver high-quality patient care in a Residential Aged Care facility using leadership skills and advanced knowledge relevant to the practice setting.
To achieve this, the Care Coordinator will be required to use advanced nursing skills in planning, coordination and decision-making within the Residential, Aged and Palliative Care settings. This includes developing highly effective interpersonal and negotiation skills, as well as oral and written communication skills to build, lead and maintain strong relationships within a team environment.
Key Responsibilities:
* Lead advanced nursing practice within the Residential, Aged and Palliative Care setting.
* Plan, coordinate and make decisions using advanced skills within the Residential, Aged and Palliative Care setting.
Required Skills and Qualifications:
* Demonstrate understanding of continuous quality improvement programs and risk management within practice.
