Key Responsibilities:
• Assist clients in reaching their full potential by coordinating care services and delivering exceptional customer service
• Represent the organization at partner sites, providing information sessions and maintaining client relationships
• Develop and maintain strong relationships with clients, families, and healthcare professionals
• Coordinate care services, including assessments, goal setting, and care planning
• Maintain accurate records and reports, ensuring compliance with relevant legislation and standards
• Work collaboratively with the multidisciplinary care team to achieve optimal client outcomes
Requirements:
• Bachelor's degree in nursing, current APHRA registration, and evidence of recent professional development
• Experience in coordinating or assisting in community-based care
• Knowledge of aged care and disability legislative frameworks, standards, and funding (or ability to quickly learn)
• Proficiency in Microsoft Office
• Current Yellow Card or National Disability Worker Screening Clearance, or equivalent
• Current National Police Check
• CPR certification
• Valid driver's license and insured vehicle
• COVID-19 vaccination
