About us:
The Cairns and Hinterland Hospital and Health Service covers a large geographic area with a diverse and growing population. The Health Service provides a wide range of primary care, acute and specialist services. The Commonwealth Home Support Program (CHSP) team is part of the Cassowary Area Community Health Service (CACH) and aims to support people to remain comfortable at home independently within the the community with a focus on hospital avoidance.
About you:
In your new role you will……
* Provide effective, efficient, competent, and culturally appropriate client focused clinical care to CHSP clients as per current Commonwealth Home Support Programme (CHSP) Manual .
* Promote cooperation, teamwork and understanding in undertaking generalist and/or specialist nursing practices for effective health outcomes.
* Effectively communicate at all levels across the multi-disciplinary, multi service team to promote effective team functioning and maintain accurate, timely documentation.
