Clinical Access and Population Health Specialist
Job Description
We are seeking a skilled Clinical Access and Population Health Specialist to join our team. In this role, you will be responsible for driving population health through clinical access and communication in a call center environment.
You will be the first point of contact for patients, physicians, and hospital faculty when communicating with the organization. Your critical responsibility will be to provide unparalleled experience and support to patients and physicians in various capacities.
Key Responsibilities:
* Interact with patients virtually to assess their clinical needs and requirements to achieve and/or maintain their health.
* Evaluate patient medical concerns and initial screening of patient signs and symptoms.
* Collaborate with the physician's care team and multi-disciplinary team to guide members toward and facilitate interaction with resources appropriate for care and well-being.
* Utilize resources available to uphold the population healthcare model.
Required Skills and Qualifications
To be successful in this role, you will need the following skills and qualifications:
* Bachelor's of Science in Nursing (BSN) or Associate's degree with a commitment to obtain BSN within three years of employment.
* Current license to practice registered nursing in the State of Ohio.
* Three years recent clinical experience in a physician office, home health, critical care, and/or emergency room, or two years recent critical care clinical experience with previous clinical experience.
* Demonstrated excellence in clinical assessment using the nursing process and ability to interact effectively with populations of patients/customers.
* Knowledge of keyboard with extreme accuracy and Microsoft Office products (Excel, Word, Outlook).

This is a full-time position with a salary range of $34.74-$41.69 per hour. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers.
