About the Role
The Grade 3 Social Worker will provide comprehensive psychosocial assessment, intervention, and discharge planning for patients within the Northern Health Broadmeadows Adult Inpatient Unit. The role has a strong focus on effective discharge planning and the facilitation of engagement with appropriate community supports to ensure continuity of care.
Responsibilities
* Monitoring and facilitating the progress of people through the course of their inpatient stay from admission to discharge, with a specific focus on maintaining and establishing robust supports in the community to enhance recovery and prevent relapse.
* Working independently and assertively within and across service systems to strengthen the discharge planning process.
* Providing excellent interpersonal, therapeutic, and liaison skills to prepare a consumer for discharge and to work collaboratively with the person's System of Supports (SOS), including family, carers and other service providers such as accommodation, education, financial and legal, to enhance the supportive scaffolding for each person.
* Working within a comprehensive integrated range of services to support people with serious mental illness in north and north‐western Melbourne.
Qualifications
* Approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers.
* Compliance with all legal requirements pertaining to the position, including maintaining current registration as required by your professional organisation or other applicable Acts.
* MAASW (Acc) or AASW Accredited Mental Health Social Worker status desirable.
* Minimum of 2 years' experience in clinical practice within the mental health field, including young people experiencing mental health disorders.
* Working knowledge of community‐based organisations and human services organisations.
* Understanding of the Children & Young Person's Act, Mental Health and Wellbeing Act 2022 and other relevant legislation.
