About the Role:
The Intensive Case Manager will provide intensive case management to Aboriginal community members and families accessing services.
Key Responsibilities:
1. To provide outreach and office-based services using the Housing First Principles of Practice in a timely manner.
2. To assess individual community members' needs (and family where appropriate) and arrange, coordinate and advocate for the delivery and access to a range of programs and services.
