UPA Home Care supports older people to live independently in their own homes by providing in-home services that effectively meet their needs, while promoting independence, wellness, choice and quality of life. This is an exciting opportunity for a Home Care Coordinator to join our team on a 12-month contract.
About The Role
The Home Care Coordinator works in partnership with clients and the care team to co-design a plan of care, based on the client's goals, preferences and assessed needs, whilst promoting functional independence, wellness and lifestyles choices.
The Home Care Coordinator will liaise with internal and external stakeholders to ensure positive client outcomes within resource/budget allocations.
What You Bring
Tertiary Qualification or equivalent experience in Aged care, Community. Services or a health related discipline. Excellent communication and interpersonal skills Experience in the provision of Coordination, Case Management and Care Planning. Understanding of current Aged Care Legislation, Aged Care Principles and Home Care Standards. Understanding of Consumer Directed Care (CDC). Experience using a variety of computer programs including word, excel, email, internet, electronic client records). Current Drivers Licence. Criminal History Screening (Undertaken by UPA). First Aid (or willingness to obtain) .
