About This Role
As a Senior Social Worker within the Pathways Team, you will play a critical role in supporting complex long-stay patients and driving timely, safe discharge outcomes across Metro South Health.
Responsibilities
* Deliver high-quality, evidence-based social work services in acute and subacute settings
* Lead complex case management and discharge planning for long-stay inpatients
* Collaborate across multidisciplinary teams to remove barriers to patient flow and care
* Contribute to service development, quality improvement, and clinical education initiatives
About You
You are a skilled and compassionate social work professional who thrives in complex clinical environments and is committed to improving patient outcomes.
* Demonstrated expertise in complex discharge planning and long-stay patient management
* Strong communication and collaboration skills, with the ability to influence stakeholders
* Proven ability to prioritise and manage a demanding caseload with sound clinical judgement
* Experience providing supervision, training, and contributing to service improvement initiatives
