About the role
We are seeking an experienced Care Manager to join this current 140 bed commissioning home in Mordialloc; the home is still ramping up with only 50 residents currently. As the newly appointed Care Manager you will take control and embed the clinical governance, along with the clinical team leadership, as well as ensuring that all clinical documentation is completed accurately and in a timely manner. Other duties include;
Assessing, planning, impementing and evaluating the clinical care provided to residents.
Provide mentoring and clinical advice to the care team and ensuring all relevant legislative requirements are met.
Coordinate and oversee all scheduled audits and clinical indicators are completed in line with the schedule.
Ensuring that best practice guidelines with medications management is observed and regularly monitored.
Actively monitor the ongoing successful delivery of the Aged Care Standards.

About you
Minimum 2 years experience in a similar Care Manager role within AGED CARE
Demonstrated knowledge of contemporary clinical, quality, and education practices and aged care frameworks.
Current AHPRA Registration
Excellent people management and interpersonal skills with the demonstrated ability to coach and mentor staff.
