Due to an internal promotion, Infinite Care is currently seeking a dedicated Clinical Care Coordinator and Nurse Mentor. The successful candidate will be available to work the following shifts: Monday, Thursday, and Friday from 2:00 PM to 10:00 PM, and Saturday and Sunday from 9:00 AM to 5:00 PM.
The Role
We have exciting and fast-paced opportunities to join our Wyoming NSW Clinical Care team as a Clinical Care Coordinator & Nurse Mentor. This full-time role reports to the Facility Manager, with support from the Care Manager, and involves supervising and supporting our passionate care team. The key purpose of this role is to ensure that all residents within our Chamberlain Gardens Wyoming NSW facility receive the highest quality of clinical care. The Care Coordinator is responsible for managing care needs, ensuring documentation and assessments are up-to-date and aligned with best clinical practices.
Key Responsibilities
- Provide clinical expertise and lead the clinical and care service teams across the facility.
- Assist the Care Manager in leading care teams to meet all clinical needs of residents, including monitoring clinical indicators for best practice outcomes.
- Manage assessment, documentation, and submission of AN-ACC requirements within the facility.
- Support documentation, policies, procedures, and IT system requirements.
- Coordinate assessment, care planning, and review processes.
- Support the admission process for new residents, including assessments and consultations with residents and families.
- Ensure accurate record-keeping in accordance with organizational and legislative requirements.
- Oversee facility team rosters to ensure optimal skill mix for resident care.
- Manage and document clinical risks and promote continuous improvement within the facility.
You will offer?
- Innate empathy for residents and commitment to improving clinical care quality.
- Minimum 1 year experience as a Care Coordinator or similar leadership role.
- Availability to work Monday, Thursday & Friday 2pm to 10pm and Saturday & Sunday 9am to 5pm.
- Current unrestricted Registered Nurse Div 1 with AHPRA.
- Postgraduate qualifications in aged care are advantageous but not essential.
- Extensive knowledge of aged care clinical assessments, care plans, AN-ACC, and accreditation compliance.
- Engaging leadership style to influence and develop clinical teams.
- High computing skills for documentation and reporting.
- Strong communication and interpersonal skills, with a passion for clinical outcomes.
- Ability to manage multiple stakeholders and handle stressful situations calmly.
- Valid NDIS Workers Screening Clearance or ability to obtain it.
