Job Description:
HammondCare seeks an experienced Occupational Therapist to support Aged Care Residents living in our dementia-specific residential home. The successful candidate will have flexibility to work part-time hours and days that suit their needs.
We are looking for a visionary, dynamic, and supportive team member who is passionate about making a significant difference in the lives of older people. You will be responsible for developing and implementing innovative individualized and group programs that promote healthy aging, reablement, and best practice dementia care.
The new Allied Health Care Team will work together to deliver resident-centered, evidence-based care in an environment that reduces excess disability, promotes meaningful engagement in life, and strives for continuous quality improvement.
* You will provide effective and timely occupational therapy to residents, offering support and advice to their representatives and the HammondCare team to meet individual needs and preferences.
* Collaborate with residents, their representatives, staff, and the broader Allied Health Team to assess functional care needs, set care goals, and provide treatment and interventions to achieve the best possible outcomes for residents.
* You will participate as a member of the HammondCare team in applying the Mission and values to pursue its objectives in a person-centered, continuous quality improvement work environment.
* Participate in quality improvement projects and continuing professional development.
About You:
* Commitment to engage with HammondCare's Christian Mission, Motivation, and Mission in Action.
* A tertiary qualification in Occupational Therapy and registration as an Occupational Therapist with AHPRA.
* Practical experience or sound knowledge in Aged Care.
* A current driver's license, safe driving record, and access to your own car for travel between HammondCare sites and services.
* Effective communication and interpersonal skills (written and verbal).
* Demonstrated organization and time management skills.
* Able to work independently and as part of a multidisciplinary team.
