We're looking for an experienced and values-driven professional to lead our Support at Home team in delivering high-quality, person-centred care to older people and those living with disability in our community.In this key leadership role, you'll oversee the day-to-day operations and strategic direction of our Packaged Care team - including case managers, support staff and admin - to ensure services are delivered with empathy, efficiency and in line with the latest aged care standards and reforms.This is your opportunity to shape a service that truly supports independence, wellbeing and dignity at home, while helping your team thrive in a values-based, supportive environment.
What You'll Be Doing Leading and supporting a multidisciplinary team, including recruitment, onboarding, performance management and staff developmentCoordinating the delivery of tailored, client-directed services that reflect individual needs, goals, and personal choicesProviding expert advice on emerging sector needs, service gaps, and compliance with aged care legislation and standardsMonitoring individual client budgets and service outcomes to ensure quality and sustainabilityCollaborating on annual budgets, business planning and reporting in line with funding and legislative requirementsMaintaining oversight of client data, compliance reporting, and risk management including SIRS and mandatory reportingDriving continuous improvement initiatives and supporting innovation in care models and staff practiceFostering strong internal and external relationships across community networks, providers and agencies What You'll Bring Tertiary qualifications in aged care, health, community services or a related human services fieldProven experience in managing aged or disability services, including staff supervision and service planningDeep understanding of the Aged Care Act (2024), aged care reforms, care standards and wellness and reablement frameworksStrong leadership and interpersonal skills, with the ability to coach, motivate and unify a diverse teamExperience managing client budgets, service data, and using data to inform service improvementExcellent written and verbal communication, negotiation and stakeholder engagement abilitiesSound computer and administrative skills, including Microsoft Office and client management systemsA valid driver's licence and clear police check (no listing on the Aged Care Banning Orders Register) Why Work for Rural City of Wangaratta?
