Care Partner Role
Our clients' well-being is our top priority, and we're seeking a compassionate and dedicated individual to join our team as a Care Partner. This role plays a vital part in ensuring the health and happiness of our clients by providing support and care services.
This position involves conducting thorough assessments of individuals' needs, including their physical, emotional, and social well-being. You will develop comprehensive care plans tailored to each client's unique needs and preferences, working closely with healthcare professionals to ensure effective implementation. Regular home visits are also an essential part of this role, allowing you to assess our clients' progress and provide ongoing support.
We're looking for someone with strong empathy and communication skills, who can think critically and solve problems effectively. Your excellent organisational skills will help you manage competing priorities and meet deadlines. A current Victorian Drivers Licence and Working with Children Check (Employee) or VIT Registration are required for this position.
