The Role
Make a meaningful impact every day by supporting people living with dementia as they transition from hospital to home or into residential care.
As a Dementia Support Coach, you'll provide practical, relationship-based support, modelling and coaching for clients, carers and care staff across a variety of settings.
This position offers professional growth and the opportunity to contribute to an innovative new project.
This role is offered as a Fixed Term Contract until January **** and is based out of our Launceston office in Tasmania.

Responsibilities
Provide on-ground support and implement relationship-based strategies for clients, carers and care staff.
Model and coach best-practice approaches to help carers and aged care staff understand and support people living with dementia.
Implement recommendation plans with guidance from the Team Leader, Dementia Consultant and Senior Consultant.
Facilitate and support the trial and implementation of brokerage items for clients and carers.
Collaborate with peers to build capability across aged care teams and support quality, person-centred care.
What do we need from you?
Strong organisational and time-management skills in a busy work environment.
Ability to work independently and collaboratively.
A commitment to excellent customer service and constructive conflict resolution.
A current driver's licence and safe driving record.
Willingness to travel intrastate and interstate for up to five days at a time.
Flexibility to work varying hours based on client and carer needs.
Qualifications in a relevant field: Allied Health Assistant, Diversional Therapy, EN, Aged Care, Cert III/IV in Individual Support or Leisure and Health (highly desirable).
