Role Overview
The District Nurse Coordinator is responsible for the coordination, leadership, and day-to-day management of district nursing services within the community. This fixed‐term role ensures continuity of high‐quality, patient‐centred care during a period of leave, supporting staff, managing service delivery, and maintaining clinical standards.
Key Responsibilities
Clinical Leadership & Coordination
* Provide leadership and oversight to the district nursing team
* Coordinate daily service delivery, including patient allocation and workload management
* Ensure care is delivered in accordance with clinical guidelines, policies, and best practice standards
* Support complex clinical decision‐making and escalation as required
Staff Management & Support
* Supervise, mentor, and support nursing and support staff
* Assist with rostering, leave management, and workforce planning
* Facilitate team communication and contribute to a positive workplace culture
* Identify training and development needs and support ongoing education
Patient Care & Quality
* Ensure delivery of safe, effective, and person‐centred care in community settings
* Monitor care plans and outcomes, ensuring appropriate documentation and follow‐up
* Manage patient flow, referrals, and service access
* Address feedback, complaints, and clinical incidents in a timely manner
Operational Management
* Oversee daily operations to ensure efficient service delivery
* Liaise with internal departments and external stakeholders (e.g., GPs, allied health, hospitals)
* Contribute to reporting, audits, and quality improvement initiatives
* Maintain compliance with relevant healthcare standards and regulations
Essential
* Registered Nurse Division 1, APHRA registration
Desirable
* Relevant Postgraduate qualification
* Relevant experience in Community Nursing
