An exciting part-time chance exists (until 30 June 2025) for an experienced nurse to join the Geriatric Rapid Acute Evaluation (GRACE) team at Hornsby Ku-ring-gai Hospital. The position advertised is for a 40-hour fortnight, i.e. 5 x 8-hour shifts per fortnight. The days of week required are Thursday and Friday, plus alternate Wednesday.

Remuneration : $57.11 - $58.99 per hour

What you'll be doing
The position is required to provide a high-quality outreach service, providing care at home and aiming to avoid hospital admission for people who reside within the community, in a private residence or within residential aged care facilities. The position works closely with the multi-disciplinary team comprising of nursing (RN, CNS2, CNC2), medical (Geriatrician/Registrar), Allied Health (Physiotherapist) and Administration.
The service is provided Monday-Friday within the hours of hours. Nursing shifts are rotational and commence at 08:00am, 10:00am or 12:00pm.

Requirements
- Current Registered Nurse, registered to practice with AHPRA. Hold relevant post graduate qualifications with at least 3 years’ recent experience in the speciality of Aged Care and/or Community Nursing or such other qualifications or relevant equivalent experience.
- Demonstrated ability to work effectively and problem solve both independently and as a member of the multidisciplinary team.
- Demonstrated knowledge of and/or experience in initiating and implementing changes to improve clinical service delivery and outcomes through evidence-based practice and practice development.
- Current unencumbered NSW Driver’s Licence.
