Job Description:
The Tasmanian Health Service is seeking a dedicated Nurse Practitioner to join their team working across the North West Coast.
The successful candidate will be responsible for high level clinical leadership in the development, provision and evaluation of the Community Rapid Response Service (ComRRS) within Community Health Nursing Service North West.
* Within a primary health care framework and in accordance with Agency policy and procedures, the Nurse Practitioner will evaluate and implement changes to clinical practice.
* They will initiate research and quality improvement activities.
* Act as a leader, advisor and mentor for other nurses and clinicians to ensure high level clinical excellence in the field of chronic, complex care.
The Team
The ComRRS NW is a community-based service designed to help clients avoid hospitals by collaborating closely with their GPs to provide essential care in the home.
Led by Nurse Practitioners, the service operates seven days a week, delivering timely, complex and client-centered care throughout the North West Coast of Tasmania.
