Job Description:

As a Care Facilitator, you will play a vital role in supporting older adults to live safely and comfortably in their own homes.

Your responsibilities will include:

* Conducting home visits to assess clients' needs and provide assistance
* Collaborating with healthcare professionals to develop and implement care plans
* Empowering clients to make informed decisions about their care
* Documenting client information and progress

Requirements:

* Bachelor's Degree or Diploma in Community Services (Service Coordination) or equivalent experience
* Valid driver's license
* Intermediate computer skills
* Excellent time management and organizational skills
* Proven experience working in aged care or transferable skills
