Are you ready to make a real impact beyond the hospital walls? As a Clinical Nurse with the Community Hospital Interface Program (CHIP) in Kingaroy, you'll lead the way in helping patients with complex needs transition safely home. This is your chance to work closely with a dynamic, multidisciplinary team where your expertise, leadership, and compassion will shape better outcomes and stronger community connections.

What Your Day Will Look Like….

You'll be at the heart of care coordination—liaising with clinicians, patients, families, and community services to develop and implement tailored care plans. You'll use your clinical expertise and leadership to assess needs, solve problems, and remove barriers to discharge, ensuring a smooth transition from hospital to home. You'll guide and support nursing staff, contribute to service improvement, and act as a key voice in care planning meetings. Each day offers the opportunity to create real, lasting impact while working in a supportive team that values collaboration, initiative, and patient-centred care.
