We are seeking a highly skilled and compassionate Registered Nurse to join our Dementia Service team in the Central Coast Local Health District. This position is a part-time maternity relief role, offering an opportunity to make a meaningful impact on the lives of patients, carers, and families living with dementia and related cognitive impairments. As part of a dynamic, multidisciplinary team, you will be instrumental in providing high-quality care to individuals with Behavioural and Psychological Symptoms of Dementia (BPSD), ensuring the delivery of optimal care across hospital and community settings.
In this role, you will be responsible for delivering expert nursing care to patients with BPSD in the community and hospital settings, facilitating smooth transitions for patients being discharged to the community, including Residential Aged Care Facilities (RACFs). You will conduct thorough assessments, develop and implement care plans, and collaborate closely with patients, carers, and the broader healthcare team. Your role will involve regular documentation of care, maintaining high standards of clinical communication, and contributing to quality improvement activities to enhance patient outcomes.
About You
* You will have a current authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
* You have experience using advanced clinical skills in assessment and care planning for people with Behavioural and Psychological Symptoms of Dementia (BPSD), including recent clinical experience in managing BPSD.
* You have strong communication skills and can effectively communicate with patients, families, and the healthcare team through the use of information technology and strong written, verbal, and interpersonal skills.
* You must be able to work a 7-day roster (includes weekends).
* You have a current Australian Drivers' Licence and willingness to travel to various sites across the Central Coast Local Health District.
Where You’ll Be Working
You will be working within the Directorate of Community, Chronic and Complex Care, where the Dementia Service team is based. This service operates across multiple settings within the Central Coast Local Health District, including hospital wards and Residential Aged Care Facilities (RACFs). You will be required to work collaboratively with a dedicated team of healthcare professionals, including doctors, social workers, and allied health staff, to support patients and their families throughout their healthcare journey. The role offers a dynamic and supportive working environment, where the focus is on patient-centered care and continuous professional development.
