* Monday - Friday 7am - 3pm shifts only - no weekend work required
* 85 bed Aged Care facility in heart of Shepparton

Role Overview
As the Clinical Care Coordinator, you will be the driving force behind the home's clinical service delivery. You will lead by example, support and educate care staff, and ensure high standards are consistently met in line with the Aged Care Quality Standards.
The role is well-suited to someone who values connection to elderly care and a hands‐on leadership approach.
Key Responsibilities
* Manage care planning, compliance, and audit processes
* Work as 2IC to Facility Manager
* Maintain thorough documentation, reporting and strong understanding of SIRs
* Act as a clinical resource and educator for staff
* Proven capability and experience in leading and managing a team
* Drive a culture of continuous improvement
About You
You are an experienced aged care professional who leads with empathy, integrity, and clinical expertise. You will bring:
* A nursing degree with current AHPRA registration
* Minimum 3 years' experience as CCC or 5+ years exp as RN Div 1 in Residential Aged Care
* Previous experience in a leadership or coordination role within aged care
* Solid understanding of AN-ACC funding and care documentation
* Strong interpersonal and communication skills
* A genuine passion for improving the lives of older people
