Job Description: HIP Complex Care Services Combined Occupational Therapist - Care Coordinator or Occupational Therapist within the Complex Care Team Job Description: 0.63 FTE (24 hours per week) Fixed Term from 28th July 2025 until 17th October 2025 Challenging and rewarding work environment Fitzroy location About the Role This position has two components – Occupational Therapy and Care Coordination, but due to the fixed term nature of the role, the role could be altered to focus on the Occupational Therapy component. Complex Care Occupational Therapist: As an Occupational Therapist, you will provide assessment, intervention and management to achieve optimal functional recovery, encourage self-management and promote empowerment for clients to improve their health outcomes, wellbeing and quality of life. You will work with clients experiencing safety, functional, medical and environmental issues that impact on functional independence. You will work with clients in the community with multiple health issues, comorbidities and complex care needs that impact safety, function, independence and occupational performance. Management of clients may involve home assessments, prescription of equipment, falls prevention, pressure care, functional assessments and retraining in activities of daily living Complex Care Coordination: As a Care Coordinator you will provide person-centred care coordination for eligible, high-risk clients living with chronic disease, comorbidities, complex aged care and other health and wellbeing needs. You will conduct comprehensive wholistic health assessments, coordinating all aspects of health and social care needs, monitoring achievement of client goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services. Interventions will optimise clients’ safety, wellbeing and self-management skills The classification for this position will be VF6-VF9 ($90,235.60 - $103,724.40 per annum, pro rata). Complex Care consists of multidisciplinary teams based at SVHM Fitzroy and at North Richmond Community Health Hub. The key aim of the CC/CD Service is to improve health outcomes for people who either frequently present, or who are at risk of presenting to hospital, because of complex medical and or social issues. Clinical service delivery is undertaken in the clients’ own home and their local community.
