Location: Broken Hill, NSW Employment Type: Full-Time Contract Salary: Up to $106,000 p.a. About the Role Reporting to the Clinic and Community Coordinator, you’ll play a vital role in providing patient centered care across both clinic and community settings. Your responsibilities will include: Coordinating and assisting with treatments and care activities in a general practice clinic. Conducting health assessments, wound care, chronic disease management, vaccinations, procedural support, and triage. Delivering services in the community, including home visits, domiciliary care, and follow-up for Aboriginal people aged 15 living with chronic conditions. Working collaboratively with Aboriginal Health Practitioners and trainees using a case management model. Building lasting, trusting relationships with patients, families, and GPs.
