affordable, accessible, nurse-led care.
About the Role
Reporting to the Clinical Director, you will provide holistic, end to end care within a
nurse led primary health model
* Ensures seamless
continuity of care
across acute, chronic, and preventive services
* Develops and oversees
chronic disease management programs
such as diabetes, and cardiovascular
* Builds strong
community partnerships
with hospitals, aged care, and local health networks
Skills and Requirements
* Experience as a Nurse Practitioner and current
AHPRA registration
* Demonstrated clinical experience in primary health, chronic disease, or urgent care
* Broad experience in multiple facets - such as women's health, diabetes management and preventative health
* Familiarity with Medicare item optimisation for NP models

* Permanent position with full entitlements of (sick and annual leave)
* Competitive salary: up to $155,000 pa + super (pro-rata, depending on hours worked and experience)
* Options for part-time or full-time hours (20–40 per week) set your own schedule
* Working during business hours - Monday to Friday initially - view to extend as business grows
