About the Opportunity Access Health and Community is seeking an experienced and compassionate Physiotherapist to join our multidisciplinary team at the Yarra Ranges Mental Health and Wellbeing Local. About the role As a Physiotherapist with Access Health and Community, you’ll play a vital role in improving both mental and physical health outcomes for participants, their carers, and families. Embedded within a collaborative multidisciplinary team, you will deliver holistic, participant-centred care that addresses the connection between physical conditions and mental wellbeing. Your responsibilities will include conducting comprehensive physiotherapy assessments and diagnoses, delivering evidence-based and goal-driven treatments, and empowering participants through the development and implementation of personalised Wellbeing Plans. You will also act as a key advocate, ensuring access to appropriate community and specialist supports, while engaging in local health promotion and education initiatives. This is a unique opportunity to shape the future of community-based mental health services by integrating physiotherapy into a biopsychosocial model of care that truly puts people first. This is a Permanent Part-time position (0.8 EFT -4 Days) reporting to the Team Leader of Mental Health & Wellbeing Local. What you will be doing Conduct comprehensive physical health assessments and develop tailored interventions Provide 1:1 physiotherapy sessions promoting movement, strength, and wellbeing Monitor participant progress and adapt treatment approaches as needed Work alongside the Exercise Physiologist to deliver exercise and lifestyle programs Facilitate groups that promote physical and mental health Advocate for participant needs and link them to additional services where appropriate Contribute to quality, safety, and continuous improvement initiatives. What you will bring Key responsibilities You are an experienced Physiotherapist who is passionate about holistic care and understands the complex relationship between mental and physical health. You bring clinical expertise, a compassionate approach, and a commitment to participant-led practice. Recognised tertiary qualification in Physiotherapy and current registration with AHPRA Eligibility for membership with the Australian Physiotherapy Association Strong knowledge and experience in evidence-based physiotherapy assessment and treatment Understanding of recovery-oriented and trauma-informed mental health practice Experience working holistically with participants considering intersectionality Ability to work across multiple community settings, including participant homes Basic skills in Microsoft Office and willingness to learn new technology platforms. Compliance Requirements: National Police Check, Working with Children Check, NDIS Worker Screening Check Evidence of the right to work in Australia and a valid Driver’s Licence Completion of NDIS Worker Orientation Module (free online).
