Description About the role Brisbane North PHN is seeking a Clinical Nurse to join Team Care Coordination (TCC). TCC provides support and education to help improve the health literacy and self-management of clients with chronic disease in the Brisbane North catchment area. It aims to assist clients to continue to live well in their own homes in collaboration with General Practice, Community Service Providers and hospital clinicians. This is a great opportunity for a General Practice Nurse looking for a change! This is a .8FTE (30 hours per week) and a Fixed Term Contract until 30 June 2027 Provision of phone, telehealth and face-to-face assessment to deliver comprehensive coordination of health, community and support services for people with chronic health conditions Providing ongoing support to clients and their families during an episode of care to review the delivery of the health, community and support services for clients enrolled in the team care coordination program Work collaboratively with program referrers and community services to improve the clients care and quality of life. This will include GPs, primary health staff, Queensland health, advocacy services and non-government organisations Skills And Experiences About you and Qualifications and Experience Registered Nurse with AHPRA registration Recent clinical experience with clients with chronic health conditions High level of problem solving and decision making in order to apply appropriate interventions in complex situations Well-developed communication skills with the ability to engage effectively with a wide range of stakeholders Ability to be able to work independently and within a high performing team Why Join Us?
