Key Responsibilities:
* Liaise with clients, family members, staff, and stakeholders to ensure services align with clients' needs, goals, and preferences.
* Assessment, planning, and review of care plans, with a focus on client-centric and consumer-directed services.
* Manage inside and outside sales inquiries, as well as develop and maintain new and existing referral sources.
Qualifications and Skills:
* Demonstrated knowledge and experience in coordinating consumer-directed care services for Government-funded HCP clients.
* Familiarity with the Aged Care Act.
* Advanced communication skills and the ability to model a positive culture through professional service.
* Understanding of the changing and diverse needs of clients.
* Reliable fully insured vehicle and valid driver's license.
* Australian National Police check.
* First Aid and CPR certification.
* Experience in managing staff.
* Professional Indemnity Insurance.
* Excellent networking and consultation skills to develop new referral sources and relationships with external organizations.
* Experienced in assessing and planning care needs within the community.
