We're currently recruiting for a community-based Care Partner (Clinical or Non-Clinical) to join our Sunshine Coast team, with the position based in Bundaberg.Work in a supportive, flexible, and professional environment where your values and skills are truly appreciated.
Key Responsibilities Conduct in-home assessments of participantsProvide ongoing case management, supporting both clinical and holistic needsMonitor participant wellbeing and update care/case/health plans accordinglyDocument and track participant goals, progress, and outcomes using person-centred, wellness, and reablement strategiesManage participant budgets (Government Funding) and ensure compliance with documentation and reporting standardsLiaise with ACAT, My Aged Care, PRODA, and other regulatory bodiesProvide support, training, and guidance to support staff and colleaguesAssist with service scheduling and respond to urgent care placements or changes(Clinical Care Partner Only): Occasional wound care dutiesAdvanced computer and administration skills (CRM proficiency required)Excellent multitasking, prioritisation, and problem-solving abilitiesProven ability to work independently and as part of a teamPrevious experience working to KPIs and tight deadlinesCurrent CPR/First Aid CertificateCurrent and satisfactory:Criminal Record CheckNDIS Screening CardWorking With Children CheckValid Australian Driver's Licence Desirable Qualifications Previous experience in home care packages, community care, or aged care/health sectorUnderstanding of Home Care Package Program guidelines and complianceClinical Care Partner:Registered Nurse (AHPRA) orEnrolled Nurse (AHPRA) What We Offer Company vehicle available for work purposesLaptop and mobile phone providedOngoing training and professional development opportunitiesAccess to free Employee Assistance Program (EAP)Staff recognition and wellness programsSupportive, office-based local teamFlexible work environment Ready to Join Us?
