About The Role
The Nurse Unit Manager – Preventative Integrated Care Service (PICS) is a registered nurse who will lead the PICS program strategically and operationally and be responsible for the reporting of the key performance indicators for the programs. The position is designated as the single point of accountability for the operations of the PICS program and works alongside the PICS Team Leader role within the context of the multi‐disciplinary team. This service addresses a gap for a target population of high‐risk chronic condition patients requiring specialist intervention and coordinated care in their most vulnerable periods to improve quality of life, avoid unplanned admissions, prevent possibly preventable hospitalisations (PPH) and reduce length of stay.
About You
* Ability to deliver contemporary and evidence‐based services across community and primary care working in a collaboratively to support integration and innovation across sectors.
* Ability to operationally manage across professions and multidisciplinary team environment to ensure effective service delivery and quality outcomes.
* Proven ability in the application of highly developed interpersonal, problem solving, negotiation and communication skills, relationship development and maintenance and change management skills.

* Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16.
