**About the role**
The GPHU Prevention and Population Health Program Officer will work closely with and support the Ararat based Prevention and Population Health (PPH) Team (Grampians Pyrenees Goldfields) to plan, establish and collaborate deliver programs to improve health and wellbeing outcomes for the catchment communities.
PPH Program Officers core duties are:
- Supporting Stream Leads to facilitate effective initiative that focus on collective outcomes and interventions
- A partnership approach - collaborating with cross-sector agencies to facilitate and coordinate resources and embed place-base prevention activities that are aligned with state-wide policies and priorities
- Community engagement - with diverse populations and organisations
**Key Responsibilities**
- Support a range of PPH team functions and activities including PPH planning, engagement, team-building, monitoring and evaluating, and reporting about activity and outcomes as required. Obtain assistance where required in undertaking activities and influence others in achieving objectives
- Collate data and other information about process and outcome measures in relation to Streams of work and contribute to drafting, editing and finalising reports about activity and outcomes to meet reporting requirements of funding agencies or for benchmarking with peer teams
- Support stakeholder engagement and communications across PPH programs and streams of work
- Coordinate with other PPH Officers to support each other in times of high demand and for leave coverage. Undertake supervision of technical tasks or having responsibility for a specified activity across the organisation
- Support and work as team members with PPH advisors to map and monitor Department of Health priority and local evidence-driven streams of work across the Grampians region, including by engaging with partner organisations to share information about activities and progress as required
- Support periodic team reflection and review of progress as well as to identify and communicate innovative solutions to complex matters affecting team goals
**Skills and experience**
- A tertiary qualification in Population and Public Health or relevant field, or equivalent relevant experience
- Excellent communication skills (written and verbal)
- Demonstrated energetic, team-focused and flexible approach to work tasks with evidence of capacity to initiate and self-direct, when appropriate
- Demonstrated proficiency in leading and supporting others in project implementation
- Knowledge of the community-based health, welfare and primary care service sector in the Grampians Region
