About the Role

We're seeking Lifestyle Facilitators who can build meaningful connections with people with disability and walk alongside them as they live a self‑directed, engaged and fulfilling life.
This is a support role delivered in people's homes and throughout the community, where you'll walk alongside people with disability as they build independence, confidence and connection. Lifestyle Facilitators help people participate in daily life, strengthen their independence, and build a sense of belonging. You'll bring empathy, reliability and creativity and we'll provide a thorough induction, values‑aligned supervision, and support to grow your confidence and capability.
What You Will Do
Person‑Centred Active Support
* Work with people (not for or to them) to build skills, confidence and independence.
* Support people to participate in daily living tasks at home and in the community.
* Promote dignity, autonomy and choice in all aspects of support.
* Respond flexibly to individual goals, routines, communication preferences and cultural identity.
Community Participation & Relationship Building
* Support people to join community activities, meet new people, and build social connections.
* Facilitate meaningful experiences that reflect the person's interests, strengths and aspirations.
* Encourage freely given relationships and support communication and social skill development.
Health, Wellbeing & Safeguarding
* Provide high‑quality personal care, meal support, medication prompts and daily living assistance.
* Embed safeguarding principles and uphold the human rights of people with disability.
* Maintain a safe environment through risk identification, WHS practices and duty of care.
* Administer First Aid/CPR during emergencies within scope of training.
Values‑Led Practice & Documentation
* Model the values of Equity Works in every interaction with fairness, empathy, cooperation and integrity.
* Complete person‑centred notes, incident reports and documentation to a professional standard.
* Maintain confidentiality, privacy and respect at all times.
* Seek feedback from people and families to support continuous improvement.

* Physical capacity to meet the practical requirements of the role (bending, standing, assisting with equipment etc.)

Essential Requirements
* Relevant experience and/or qualifications in disability, community services, aged care or similar
* Current NDIS Worker Screening Check and Blue Card (or willingness to obtain)
* Current First Aid and CPR
* Valid QLD Driver Licence
* Access to a reliable, safe vehicle (comprehensive insurance and safety certificate required)
* Completion of the NDIS Worker Orientation Module (or willingness to complete)
