As the Community Nurse, you will provide direct person-centred nursing care and individual case management to patients in the community setting at Port Lincoln. Working with clients in the community where there are multiple complexities and diverse cultural backgrounds, you will provide health promotion and education to patients and carers to improve the health outcomes of each individual. It is expected that you will provide support and guidance to newer or less experienced staff, Enrolled Nurses, student nurses and other workers providing basic nursing care.
