Southcoast Behavioral Health, located in Dartmouth, Massachusetts, is a premier mental health treatment center with 192 beds. We serve pediatric, adolescent, adult, and geriatric patients, as well as adults with co-occurring mental health and substance use disorders.
PURPOSE STATEMENT:
Manage the client's/patient's/resident's cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to clients/patients/residents.
Responsibilities
ESSENTIAL FUNCTIONS:
1. Provide case management and care coordination services for clients/patients/residents in the facility.
2. Coordinate continuum of care activities for assigned clients, ensuring optimal resource utilization, service delivery, and compliance with external agencies and referral source requirements.
3. Conduct psychosocial assessments and facilitate individual, family, and group sessions.
4. Coordinate treatment and discharge activities as needed.
5. Develop, implement, manage, and evaluate client care plans to ensure quality care.
6. Collect, analyze, and provide clinical data to the treatment team.
7. Convey medical criteria and clinical information between insurance providers and the treatment team.
8. Correlate clinical data for the business office as required.
9. Address issues or activities relevant to the treatment team promptly.
Qualifications
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
- Bachelor's degree in Social Work, Counseling, or a related human service field required.
- One or more years of experience in a behavioral health setting preferred.
- Experience with the patient population served by the facility preferred.
- Experience in case management and/or utilization review preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
- Current license required within the state of operation, if applicable.
- CPR and de-escalation/restraint certification required (training provided upon hire).
- First aid certification may be required based on state or facility requirements.
