**About the team**
The HOPE program is a Victorian Government initiative which was implemented to provide enhanced support and assertive outreach to individuals who have recently been discharged from an emergency department or medical ward following an attempted suicide, or serious planning or intent.

The HOPE program provide intensive, person-centred support with is tailored to the unique needs of the individual for up to 12 weeks. These individuals are supported to address factors that contribute to stress in their lives, which can include assistance with employment, mental health support, and referral to range of other community supports. The HOPE program also works with the families, carers, or friends of the individuals who have attempted suicide, to provide support and link them in with appropriate services.

**About the role**

The Psychosocial Support Worker will work collaboratively with the multidisciplinary team, which includes clinical staff and peer workers, assisting participants to navigate services, offer linkage and referral, provide practical support when required, offer support to carers and family and develop and assist with safety planning.
- Establish supportive and respectful relationships with people using the HOPE program and support them with their recovery goals.
- Collaborate with consumers and their chosen support network to develop and implement psychosocial support plans and suicide safety plans.
- Provide practical assistance to support consumers to attend appointments/meetings/activities that form part of their psychosocial support plan.
- Identify and engage other key community agencies, assisting consumers with referrals and linkages.
- Support consumers to better manage their distress using brief supportive intervention strategies.
- Provide support, information and education to participants families, carers and significant others.
- Work collaboratively with the multi-disciplinary team and participate in team and clinical meetings.

**Skills and experience**
At least one of the following education standards or level of practical experience:

- Diploma level qualification in a relevant field including but not limited to: Alcohol and other drugs; Community Service; Disability; Mental Health; Youth Work or similar.
- Students currently completing university degrees in nursing, social sciences or allied health including: Occupational Therapy; Psychology; Social Work or similar.
- A minimum of 2 years full-time experience in working with, and providing support to persons experiencing psychological distress; mental ill health and/or suicidal ideation.
