About The Role
Northern NSW Local Health District is seeking a Palliative Care Clinical Nurse Specialist Grade 2 (CNS2) to join their team in Ballina. This role works with members of the palliative care nursing team across the inpatient and community settings to provide a consultative and liaison focussed service aimed at supporting GPs, Community Nurses and the Residential Aged Care In-Reach Team in the assessment, care and management of patients with life limiting illnesses. The aim is to facilitate a coordinated, high quality palliative care service to the local community, with a case coordination model that involves generalist community nurses in providing direct clinical care. The aim is to use a multidisciplinary approach to care, with the end goal of improving quality of life for those patients and their carers.
