About the Role
We are seeking a skilled Clinical Care Coordinator to join our team. The successful candidate will be responsible for coordinating clinical services in order to support clients in their own home.
The primary point of contact in supporting clients and staff to ensure services are delivered efficiently and provide or clients with the best possible outcome.
This role involves daily coordination of care for new and existing clients of the SA Health Community Care Program, acceptance of referrals including the Metropolitan Referral Unit (MRU), and ensuring all client information is available to commence service delivery within the required service delivery timeframes.
Key responsibilities include:
* Daily coordination of care for new and existing clients of the SA Health Community Care Program.
* Acceptance of referrals including the Metropolitan Referral Unit (MRU) and ensuring all client information is available to commence service delivery within the required service delivery timeframes.
* Identify clients in the clinical programs that are eligible for other Government funding streams and liaison with the referrer, MRU or healthcare professional for transfer of care.
About You
To succeed in this role, you will need:
* Bachelor of Nursing qualification.
* Experience in Acute Nursing.
* Community nursing experience.
* Full Drivers Licence.
* Previous experience managing service contracts.
* Previous experience managing staff.
* Strong problem solving and time management skills.
* Effective communication skills.
* Computer skills.
* Ability to work over 7 day roster.
* Logistical experience (desirable).
