Nexia is expanding its Support at Home services and is looking for an experienced Care Manager to join our small, close-knit team. This part-time role (4 days/week) offers a hybrid work arrangement, combining work from home, community visits, and time at our New Gisborne office.
You'll work closely with our clients, Service Coordinator, and Clinical Lead to deliver person-centred care. You'll monitor and action client intake daily, conduct ongoing in-home assessments, and update care plans continuously. You'll also support community engagement activities and participate in a shared after-hours on-call roster.
While scheduling is managed by the Service Coordinator, you will provide backup support when needed. You'll also be well supported by our Clinical Lead, who takes a hands-on role in clinical assessment and oversight.
What we're looking for:
* Minimum 2 years' current experience in Home Care Packages or aged/community care (Allied health or nursing background highly regarded)
* Sound knowledge of Support at Home reforms
* Onboarding new clients experience is a must
* Experience with Sandwai and Resident Select desirable
* Knowledge of Aged Care Quality Standards
* Strong communication, organisation, and collaboration skills
* Current driver's licence and access to a vehicle
* Australian working rights and National Police Check valid within 6 months
