Aged Care - Intake Officer

Salary Range : $ per hour + superannuation + salary packaging
About our role
This position is permanent part time working 24 hours over 3 or 4 days a week. An exciting opportunity exists to join to rapidly growing Aged Care Services Department at WDHS as an Intake Officer. This position is to support the ever-growing Support at Home and Community Support Services teams. The role of Intake Officer is to oversee the intake process of potential Support at Home & CHSP participants by completing an assessment to identify their needs and goals in order to support them to remain living safely and independently at home.

About you
* Previous experience in aged care, community services or a similar role
* Understanding of SaH, CHSP, HACC-PYP and My Aged Care frameworks, policies and procedures are beneficial but not essential
* Excellent interpersonal and communication skills, strong organisational and time management abilities, IT proficiency
